Prior to Your Visit
What to bring to your appointment
To make your visit as smooth as possible, please have the following ready before you arrive.
- ✓New patient paperwork — New patients and returning patients with a new injury: please complete new patient paperwork before your first visit. Visit the Forms section of this website to get started.
- ✓Therapy prescription or referral — Bring your physician's therapy prescription to your first visit. This allows your therapist to build a program tailored to your specific condition, diagnosis, and your doctor's recommendations.
- ✓Photo ID — A valid driver's license or current photo ID is required at every visit.
- ✓Insurance card — Bring your insurance card to every appointment. Co-pays are collected at check-in. As a courtesy, we verify and bill your insurance for therapy visits and any durable medical equipment provided in our office. Contact your carrier before your visit for complete benefit information. We accept cash, personal checks, debit cards, Visa, and Mastercard.
- ✓Appropriate attire — Wear comfortable clothes suited to exercise and movement — shorts, sweats, t-shirts, and supportive shoes. For knee conditions, shorts are recommended so your therapist can properly evaluate and treat the area. For shoulder conditions, a tank top or sleeveless shirt works best.
Pre-register before your visit. You can complete your health history through our patient portal (e-Chart) up to seven days before your appointment. After your visit, office summaries are available there as well. Access e-Chart at ehrweb.tristateortho.com/phxportal

Print and complete these forms before your visit to save time at check-in. New patients should bring the Health History Form and HIPAA Notice on their first visit.
